- Bring in person to the Theater and give to Tom Gaffey
- Mail the form to the Phoenix (201 Washington St. Petaluma, CA 94952)

PLEASE NOTE:
This is not a contract. This is the initial rental application. Submission of this application does not guarantee that dates are on hold nor that the event is confirmed. Events are confirmed once a finalized contract has been signed by both the Phoenix and the event promoter.
